Your recent blood tests show that you have a condition called Chronic Kidney Disease (CKD).

This is not something to be alarmed about. It is a relatively common condition which can be associated with getting older.

This does mean regular monitoring of your kidney function, blood pressure and other blood tests markers, which you will receive reminders for when you they due, or when you order your repeat prescription.

We do need a urine sample test from you to check the level of protein in your urine. This should be an early morning sample, please drop this into the surgery fully labelled with your name and date of birth and writing clearly – ‘requested for CKD’.

Further information on the Chronic Kidney Disease is provided here: https://cardiffsw.co.uk/chronic-kidney-disease-ckd/

Should you have any questions regarding this please contact the surgery to book a routine telephone advice call with a GP.

Yours sincerely
