Wednesday 19th March is National Social Prescribing Day!

Social prescribers connect people with their communities, whatever their age or background, to help them better manage their health and wellbeing.

Social prescribers, or Community Connectors, take time to work with individuals to identify their non-medical needs through a ‘what matters’ conversations, helping them find the best way forward to improve their wellbeing.  This may include connecting people with groups in their community such as sport, art, gardening, but can also go beyond that to connect people to support with other social issues such as financial support, housing support.

Reasons for referrals: Addictions, Bereavement/grief, Cancer, Carers, Confidence and self-esteem, Fuel poverty, Housing repairs, Mental health and well-being, Mobility/safety, Money/debt, Nutrition, Peer support Neuro-diversity, Physical health, Sensory impairment, Social isolation, Stress, Tenancy, Training, learning and employment.
