Regular MDT meetings have been established across the Cluster. Ely/Caerau and Canton/Riverside. These are attended by GPs, Mental Health Liaison Service, Independent Living Service Cardiff & Vale, Community Resource Team, District Nurses, Practice Nurses, Care and Repair, Cluster pharmacists, ACE wellbeing Coordinators,

Cases are brought to the meeting by any of the team members who feel that an individual is vulnerable and in need of support. Members of the team discuss possible solutions and follow up arranged as required to monitor outcomes. Feedback is highly positive and team members feel empowered to support people with complex needs through the integrated team approach.
