The Integrated Care Hub is based in Lansdowne Surgery and is supported by administrators, Cluster Pharmacists, CRT OT. The hub team arrange the Cluster MDT meetings and have a key role in identifying need for support for people following discharge from hospital. The hub works closely with practices who identify individuals who may be in need of support.

Staff at the hub access discharge summaries from patients from all cluster practices and contact patients and identify those in need of support, which will be arranged through coordination with health and social care services as well as community connections. The Cluster Pharmacists deal with all medicine reconciliations for all the Cluster practices that are on the discharge advice letters.
